The Graceful Place to Grow accepts enrollment throughout the year. Contact us for more detailed information about enrollment.

The following is a list of the forms we require for enrollment. Once accepted, families will need to complete the following:

• Child Care Enrollment Form: To be officially registered, a family must submit a completed enrollment application with the appropriate registration fee of $100. Registration fees are nonrefundable.

• Child Medical Examination Report: Please ask your pediatrician to return this medical exam form with a vaccination record.

• Pickup Authorization: Authorize individuals not listed on the enrollment application for pickup.

• Media Release Authorization: To authorize the taking of films, video, audio recordings, photographs, and slides to be taken of your child for use on the website or in print and email publications.

• Permission Slip: We frequently take walks around the neighborhood and a permission slip form is required for enrollment.
